First Visit

Please assist us at the time of your initial visit to the office by providing the following information:

• referral slip and x-rays from your referring dentist ( if valid)
• a list of medications your are currently taking
• if you have dental insurance, please bring any information with you to your appointment.

Please note:

• All patients under the age of 16 must be accompanied by a parent or guardian at the consultation appointment.
• Please notify the office if you have a medical condition or concern prior to treatment (e.g. artificial heart valves, or joints, heart murmurs requiring pre-medication, poorly controlled diabetes, or hypertension.)
• We have a cancellation policy. We require two business days notice to avoid a fee
• If you are parking your car, remember to purchase a ticket for $3.00 – please bring up the stub so we can reimburse you.
